Student Enrollment

Any parent/guardian who wishes to enroll their child must do so between 9:00am-11:00am Tuesdays through Thursdays. All enrollment documents (given in the main office or downloadable below) must be completed in addition to bringing the following documents in order to proceed with enrollment:

  • 2 proofs of address (utility bill, lease, cable bill, etc.)
  • Birth certificate
  • Transcript from previous school (if applicable)
  • Social security card (if applicable)

Please note: Unless you are a member of one of our CTE programs or joining our ESL program, you must live in our attendance area to enroll at Sullivan. Additionally, all enrollments are subject to principal approval.


All of the following forms are required to enroll your student at Sullivan High School.  Please download and complete the forms.  Note that students who are new to CPS (the Chicago Public School System) must fill in two additional forms to complete their registration.
